Responsibilities

Receive referrals from public, private and insurance companies and determines appropriate staff to be scheduled
Communicates directly with clients to determine scheduling requirements
Confirms services with client, family and Ontario Health atHome
Assists with recruiting/onboarding initiatives
Accurately provides client information and travel information to scheduled team members
Ensures and maintains continuity of schedule for every client
Resolves and documents day to day issues in a timely manner.
Communicates with all stakeholders scheduling changes
Ensures client safety
Prepares incident reports as required and ensures
Daily tasks including filing, faxing, reading and processing files
Hours per week: (8:30am-4:30pm, Monday to Friday),Some nights and weekend on-call 40
